Prevalence of Cardiorenal Syndrome among Admission Cases undergoing Transcatheter Valve Procedures and its Association with Hospital Outcomes

Abstract
Background
Chronic kidney disease is a common comorbidity among patients with valvular heart diseases (VHD) undergoing transcatheter valvular procedures (TVP). However, the prevalence of cardiorenal syndrome (CRS) has not been established among patients with VHD undergoing TVP.
Methods
National Inpatient Sample database was queried from 2016 to 2019 to identify admission cases with VHD who had undergone TVP. Need to in-hospital hemodialysis (HD), mortality rate, length of stay, and total hospital charges were compared between different TVPs and stratified based on CRS.
Results
Data of 38,954 TAVR cases, 216 TPVR cases, 2,762 TMVR cases, and 56 TTVR cases was analyzed. The prevalence of CRS was significantly higher among admission cases undergoing TVP compared to those not receiving these procedures. Among admission cases undergoing a TVP in the left-sided heart valves, the presence of cardiorenal syndrome was associated with a significantly higher length of stay, hospital charges, and higher mortality rate. However, such a relationship could not be demonstrated for admission cases undergoing TVP on the right-sided heart valves, probably due to small sample size.
Conclusions
The presence of cardiorenal syndrome adversely affects the hospital outcomes of patients undergoing transcatheter procedures on the left-sided heart valves.
