The Clinical and Educational Value of a Structured Transitions in Care Clinic -- Fulfilling the Quadruple Aim

Abstract

The transition from inpatient to outpatient presents opportunities for patient morbidity and mortality. The ACGME recognizes this as an education opportunity, stating that, "The [inpatient]experience should include the care of patients through hospitalization and transition of care to outpatient follow-up of the same patient in a continuity relationship". Primary care offices have struggled to maintain patient access, with many clinics having 1-2 month waits for routine visits, while also maintaining same-day access. This creates difficulties in scheduling patients for urgent outpatient visits after hospitalization. Given these barriers, our residency started a Transitions of Care Clinic to fit our quadruple aim of: Enhancing the patient experience by providing timely access to physicians who know your hospital course; improving population health by caring for our sickest patients in a timely manner with a dedicated faculty member; reducing costs by helping to reduce readmissions to the hospital; improving the wellbeing of physicians by providing a different clinical experience for both residents and faculty lead. We aim to briefly share how we logistically set up this clinic, what type of faculty needs are met by this clinic, how this clinic can be financially viable for a residency, and how it improves the inpatient experience for residents, sharing qualitative and quantitative data from our program.

Upon completion of this session, participants should be able to:

  1. Provide the educational argument to your program director for a clinic that bridges the inpatient and outpatient experience.
  2. Identify the faculty members who will best champion for a clinic that bridges the inpatient and outpatient experience, both to improve patient experience and their own wellbeing.
  3. Make the financial case to your medical director for a clinic that bridges the inpatient and outpatient experience.
