Reversal Failure: Cardiovascular Compromise Following Protamine Sulfate Administration Requiring VA-ECMO Support

Abstract
A 72-year-old female with history of minimally invasive AVR presented with endocarditis and aortic root abscess. Patient underwent Bentall procedure. After the procedure, patient was reversed with protamine sulfate. Approximately five minutes after protamine administration, there was global cardiac dysfunction evident on TEE. Intra-aortic balloon pump was placed and cardiac function improved. She had been re-heparinized with planned reinstitution of cardiopulmonary bypass. However, it was deemed unnecessary given improved cardiac function. Protamine sulfate was re-administered with subsequent global hypokinesis. CPB was reestablished, weaned to VA-ECMO, and admitted to the surgical ICU. We will discuss the etiology of cariogenic shock and indications for VA-ECMO.
