Intraoperative Carbon Dioxide Embolism

Abstract
We present a case of carbon dioxide embolism in a 65 male, with BMI of 47, and history of sleeve gastrectomy undergoing conversion to gastric bypass. Shortly after insufflation, the patient’s end tidal CO2 abruptly dropped, he became hypotensive, and went into cardiac arrest. The surgical team noted blood upon removal of the Veress needle. After inserting a laparoscope, puncture of the liver was identified. With this case we will discuss presentation, mechanism, and management of carbon dioxide embolism.
