Management of Cancer Pain Exacerbation with Ketamine Infusions in the Setting of Opioid Tolerance

Abstract

We present a 28-year-old female with history of metastatic breast cancer admitted for uncontrolled cancer pain. Upon initial evaluation, she was found to have new pathologic compression fractures in her thoracic vertebral bodies and lytic lesions in her cervical spine. To control her pain, her multimodal pain regimen consistent of both short and long-acting opioids in addition to NSAIDS, muscle relaxants, massage, heat/ice, and physical therapy. She was eventually placed on a ketamine infusion to effectively control her pain. This case will discuss ketamine and opioid physiology and their role in the treatment of chronic cancer pain.
