Abstract
A 7do male infant born at 38w0d presented for thoracoscopic repair of a congenital diaphragmatic hernia. Pregnancy and delivery were uncomplicated. Following birth, patient developed respiratory distress and was discovered to have large left sided congenital diaphragmatic hernia. Patient was intubated, mechanically ventilated, and medically optimized prior to presentation for surgical correction. Will discuss CDH physiology including an approach to intraoperative management of pulmonary hypertension. Will additionally discuss medical optimization prior to surgery, including consideration of ECMO as a bridge to surgical correction in patients born with CDH.
