Methemoglobinemia in a Patient With Amyloidosis: A Case Report and Treatment Outcomes

Abstract

A 65-year-old woman, with a medical history significant for amyloidosis (managed with cyclophosphamide, bortezomib, and prophylactic dapsone) atrial fibrillation, and supraventricular tachycardia post-ablation, presented to the emergency department with abdominal pain and diarrhea. On evaluation, she exhibited hypoxia and was diagnosed with a small bowel obstruction. During exploratory laparoscopy, she experienced significant hypoxia, with arterial blood gas revealing methemoglobinemia (11.2%, normal values are 0-3%). Treatment with methylene blue successfully reduced methemoglobin levels (2.4%). This case report will discuss differential diagnosis for intra-operative hypoxia and presentation, pathophysiology, and treatment of methemoglobinemia.
