ECMO-Related Compartment Syndrome: Mitigation, Risks, and Treatment

Abstract
We present a 27-year-old male with a 15-year smoking history who presented to the hospital for bilateral influenza A pneumonia with accompanying respiratory distress that required intubation. Soon after, the patient entered a state of cardiogenic shock related to newly diagnosed dilated cardiomyopathy and was placed on venoarterial extracorporeal membrane oxygenation (VA-ECMO). Two days later, the patient developed acute compartment syndrome of the bilateral lower extremities which required four-compartment fasciotomies and eventual bilateral above-the-knee amputations (AKA). In this case, we will discuss the rare complication of compartment syndrome while on peripheral VA-ECMO.
