Implications of Granulomatosis with Polyangiitis on Anesthetic Management

Abstract
A 47-year-old female with a saddle nose deformity, oronasal fistula, atrophic right kidney, chronic pansinusitis and ulcerated skin lesions presented with a painful LLQ abdominal bulge. After extensive diagnostic work-up, she was found to have granulomatosis with polyangiitis (GPA) and a spigelian hernia. She subsequently underwent successful surgical repair of her hernia. GPA is a form of necrotizing, granulomatous vasculitis that classically involves the upper airways and kidneys. This disease, although rare, should be on an anesthesiologist's radar due to the pulmonary and cardiovascular implications that it has on anesthetic management.
