Thromboembolic Prophylaxis in High Risk Patients With Contraindications to Chemical and Mechanical Prophylaxis

Abstract

A 47-year-old male with a history of heart failure, morbid obesity, and recent gastric laparoscopic band placement presented with surgical complications of bowel perforation and wound dehiscence. Subsequently he was taken for a revision surgery and Roux-en-Y gastric bypass. This patient, who is at high risk for venous thromboembolic event (VTE), had several contraindications for VTE prophylaxis including: recent major surgery, active bleeding, and chronic weeping venous stasis ulcers of the bilateral lower extremities. We will discuss primary and secondary VTE prophylaxis in general surgery patients, including alternatives for prophylaxis in high risk patients with contraindications to chemical and traditional mechanical options.
