Perioperative Management of a Patient With a CO2 Embolism

Abstract
This case discusses a healthy 57 yo female who presented for a robotic hepatic cyst fenestration. During inflation of the abdomen intraoperatively, EtO2 decreased from 31 to 13, with bradycardia in the 30s and eventual asystole necessitating CPR. ROSC was achieved after 5 minutes, and arterial line and CVC were placed with aspiration of venous air. Intraoperative TEE showed a large amount of right and left sided intracardiac air and a PFO; raising concern for possible neurologic involvement. This case will discuss management and complications of a CO2 embolism, and proper postoperative care.
