Methemoglobinemia in a Patient With Undiagnosed G6PD Deficiency

Abstract

A 58-year-old male presented for elective valve-sparing aortic root and ascending aorta replacement. Post-op patient developed ARDS requiring VV ECMO and initiation of nitric oxide. He developed methemoglobinemia and was treated with methylene blue causing acute hemolysis secondary to likely G6PD deficiency. We will discuss the pathophysiology of methemoglobinemia, G6PD deficiency and the interaction between the two conditions.
