Differential Diagnosis Of Orthopnea And Pleural Effusions

Abstract

Orthopnea is a common chief complaint amongst patients seeking care across all specialties. Our patient is a 16-year-old female who initially presented to her primary care physician with complaints of difficulty lying flat and worsening shortness of breath over the past month. Although her family doctor initially dismissed her symptoms, her family advocated for a chest x-ray, which revealed a large right sided pleural effusion. Patient was admitted to the PICU for acute respiratory distress and further work up. We will discuss the differential diagnosis for orthopnea, pleural effusions, and relevant work up to reveal the etiology.
