Massive Transfusion: a Discussion of Ethical Principles

Abstract
15-year-old female with multiple gunshot wounds to the LLE, buttocks and pelvis, hemodynamically unstable with asystole upon presentation. CPR and resuscitative thoracotomy in trauma bay preceded emergent exploratory laparotomy during which multiple large vessel injuries were oversewn and patient brought to ICU for warming and resuscitation before further surgical intervention. Despite receiving 7 coolers of blood, she remained unstable and coagulopathic needing proportionally more platelets. At this time there were 4 total units of platelets left for the entire hospital and ethical principles were at odds. I plan to discuss the ethical implications of this case.
