Lung protective strategies in the PICU: High-Frequency Oscillatory Ventilation (HFOV) indications and weaning parameters.

Abstract

A five-month-old male born at 29 weeks and with a history of chronic lung disease of prematurity and persistent pulmonary hypertension presented in acute on chronic hypoxic respiratory failure. He had multiple PICU admissions for respiratory failure requiring conventional ventilation and prior use of high flow oscillatory ventilation (HFOV). During this admission, he was intubated but continued to have increasing oxygen requirements on multiple ventilator settings. Despite the addition of inhaled nitric oxide, his condition continued to deteriorate. He was transitioned to HFOV, where his oxygenation and overall course improved. We discuss the indications and weaning parameters of HFOV.
