Racial Disparities in N-STEMI Management and Outcomes in the Southern United States. A Population-Based Retrospective Analysis

Abstract
Background: Analysis focused on NSTEMI diagnosis and catheterization rates among racial groups in Southern States compared to non-Southern regions. Methods: Retrospective analysis used 2016–2020 NIS data on NSTEMI patients. Exclusions included age < 18, incomplete demographics, and 2 NSTEMI diagnoses. MLR adjusted for confounders (age, gender, income, insurance type, and comorbid conditions). Caucasians served as control group. Analysis with STATA 17. Results: Southern states, Blacks had a 21.3% lower likelihood of cath (OR=0.78, CI=0.759–0.814, p< 0.001), with similar trends for Hispanics/ Asians. Non-Southern regions, Blacks had an 8.3% reduced likelihood, while Hispanics/Native Americans also showed decreased odds. Conversely, the "other" racial group had a 27.9% higher probability of cath. Asians in non-Southern regions faced a 50% higher risk of in-hospital death, with Blacks/Hispanics also at elevated risk. Blacks had (27% and 39%) and all other groups showed higher likelihood of cardiac arrest in South. ICU admission rose for all groups except Asians in South. Conclusions: Study highlights disparities in cardiac care and outcomes in Southern regions of the US. Blacks and other minority groups consistently faced lower probabilities of receiving cardiac caths. Higher odds of death were observed for all groups in the South, except Blacks. Higher odds of death were shown for all groups in non-Southern regions. Racial minorities overall experienced increased likelihoods of cardiac arrest/ICU admissions. Results emphasize the urgent need for targeted interventions to address racial inequities in cardiovascular care and outcomes.
