Contemporary Temporal Trends and In-Hospital Outcomes in Elderly Breast Cancer Survivor Women Hospitalized With Acute Myocardial Infarction Undergoing Percutaneous Coronary Intervention: Insight From the National Inpatient Sample (2016-2020)

Abstract

Background: The interplay between oncological history and cardiovascular events in elderly breast cancer survivors (BCS) remains understudied, particularly in the context of acute myocardial infarction (AMI). This study aims to evaluate the trends and healthcare outcomes in this population undergoing percutaneous coronary intervention (PCI). Methods: The National Inpatient Sample from 2016 to 2020 was utilized to identify elderly BCS admitted with AMI and undergoing PCI using ICD-10 codes. The outcomes studied were all-cause mortality (ACM) and PCI-related complications. A multivariable analysis was performed, adjusting for demographics and comorbidities. Results: We have identified 17650 elderly BCS (median age 77, white 84.1%) from 2016–2020. Over this period, the rates of hyperlipidemia (69.4% to 76.5%), obesity (13.3% to 23.0%), and valvular disease (0.7% to 2.0%) have increased. The rates of in-hospital outcomes like ACM (3.3% to 3.8%) and post-procedural stroke (1.3% to 2.2%) increased in this population, along with LOS and healthcare costs after PCI (all p<0.001). From 2016–2020, the socioeconomic analysis showed a 2.7- fold relative increase in ACM in the lowest quartile (ptrend¼0.028) and 2.2 fold relative increase in the 3rd quartile (ptrend<0.001), with a 0.7- fold relative decrease in the 2nd quartile (ptrend¼0.01). However, the multivariable analysis did not show any significant association between in-hospital outcomes and PCI. Conclusions: The rates of comorbidities have increased in elderly BCS from 2016-2020, while the rates of in-hospital outcomes like ACM and post-procedural stroke have increased.
