Complete Heart Block Following Herpes Zoster Outbreak

Abstract

A 53 year old female with recent herpes zoster outbreak along T4-5 dermatome of her left back presented with syncope. Initial work-up was unremarkable. On hospital day 2 she developed third degree heart block, was transferred to the Coronary Care Unit, and on hospital day 3 a permanent pacemaker was placed. Further work-up was supportive of sarcoidosis or varicella zoster myocarditis as the etiology of her heart block. This case demonstrates a unique presentation of complete heart block.
