Death By Intubation: Securing The Airway In Massive PE

Abstract
A 76y.o. female with history of obesity, NIDDM and HTN presented to the emergency department with shortness of breath. CT PE revealed extensive bilateral pulmonary emboli with severely enlarged right heart. Catheter directed TPA was initiated. The following morning, the patient had increased O2 requirements and hypotension. Following intubation, the patient suffered hemodynamic collapse unresponsive to resuscitation and the patient expired. In this presentation we will discuss airway management in massive PE, and how traditional means to secure the airway, including sedation, intubation, and positive pressure ventilation may be deleterious for these patients.
