Ventilation and Perfusion Mismatch and Mechanisms of Hypoxemia in Lung Disease

Abstract

We present a 75-year-old female who required emergent surgery for an incarcerated hernia of her right groin. Her perioperative course was complicated by a history of metastatic lung cancer causing complete atelectasis of the right lung. Her arterial blood gas showed a hypercarbic respiratory acidosis with a PaO2 67 mmHg despite being on 6 liters of oxygenated nasal cannula. We will discuss ventilation and perfusion mismatch and the mechanisms of hypoxemia in lung disease.
