Patient Portal Access and the Delivery of Bad News

Abstract

This case discusses a 40-year-old female with metastatic breast cancer to the spine and pelvis that presented to the emergency department with an initial complaint of shortness of breath. CTA chest was obtained to evaluate for PE and though this was negative for emboli, there was concern for metastatic disease outlined in the radiology read. The patient received the results on an online patient portal prior to the physician personally delivering the information, upsetting the patient. This case prompts the discussion on what information is appropriate for patient portal users to receive prior to their physicians.
