A Case of Ectopic Mediastinal Parathyroid Adenoma Causing Persistent Hyperparathyroidism After Bilateral Neck Exploration

Abstract
We present the case of a 55-year-old female with ectopic anterior mediastinal adenoma. Patient failed bilateral neck exploration with persistent intra-operative and post-operative symptomatic hypercalcemia and elevated PTH. Patient had vitamin D deficiency which also contributed to some degree of elevated PTH. Patient was subsequently found to have a cystic anterior mediastinal mass on imaging which was resected and confirmed as ectopic parathyroid tissue by pathology with subsequent biochemical and clinical resolution of hyperparathyroidism. We will discuss embryonic origin and migration of parathyroid gland, potential causes of persistent hyperparathyroidism following neck exploration, and utility of various imaging modalities
