Stuck on Bypass: Complicated Left Atrial Myxoma Excision

Abstract

A 61-year-old female presents for excision of a symptomatic left atrial myxoma. Baseline TEE showed trivial MR. After removal of the mass, patient developed severe MR and LVOT obstruction, likely due to a change in the mitral valve annulus architecture, now requiring a mitral valve replacement. Subsequent attempts to separate from bypass were unsuccessful due to refractory vasoplegia with maximal vasoactive support and an intra-aortic balloon pump. The patient was ultimately placed on veno-arterial ECMO as a bridge to recovery. Unfortunately, she passed away three days later due to the low-output state and multi-organ failure.
