Accidental Drilling Of The Femoral Artery

Abstract

An 88yo female with PMH of HTN presented to the hospital for a revision left total hip arthroplasty. During the procedure, the femoral artery was inadvertently drilled through. Delay in communication led to sudden onset hypotension (40/20), and an ETCO2 as low as 23mmHg. An arterial line was placed, and the patient was resuscitated with 2u pRBC, crystalloid and colloid. Vascular surgery was called into the operating room, with hemostasis eventually being obtained. The patient was then admitted to the ICU for further monitoring. This case discussion will focus on intraoperative hemorrhage, and its ramifications and management from an anesthetic standpoint.
