The Power Of Ischemic Preconditioning: A Case Of Perioperative Resurrection

Abstract
A 65-year-old female with a complex medical history was induced urgently for a hemi-hip arthroplasty after voicing her preference to remain DNR. Moments after induction, she went into hypovolemic shock. She was resuscitated with vasopressors and fluids. After two hours with systolic BP 30-40s, her pacemaker was turned off in preparation for compassionate wean. However, native heartbeat and spontaneous respirations resumed and that evening the patient was fully oriented, back to preoperative status. Ischemic preconditioning may explain this extraordinary resurrection and I plan to discuss this, as well as the importance of perioperative code status determination.
