Abstract
We present a 29-year-old female with a history of morbid obesity (BMI 40.40) and PCOS who presented with fever and SOB after testing positive for COVID 3 days prior. She was admitted for treatment of COVID pneumonia and developed acute hypoxic respiratory failure. Despite NIPPV the patient remained hypoxic and she was emergently intubated. SpO2’s remained in the 80’s despite maximal ventilatory therapy and the decision was made to transfer for V-V ECMO cannulation. We will discuss pathophysiology of COVID-19, indications for ECMO, and management of acute hypoxemic respiratory failure.
