Access to medical care remains a significant challenge in Kent County, with transportation barriers being a key social determinant of health. The Kent County Community Health Needs Assessment (CHNA) for 2023 highlights transportation issues as a leading factor in missed medical appointments, particularly for patients with chronic diseases. The Corewell Health West Internal Medicine Residency Clinic currently does not have standardized processes to assess and address transportation issues among its patient population, leading to missed opportunities for care continuity.
