Heartburn Takes on a Whole New Meaning: A Case of Gastropericardial Fistula

Abstract

Gastropericardial fistula is a significantly rare and life-threatening condition.

Here we present a 55-year-old female with current tobacco use and prior open gastric bypass with known gastrogastric fistula and recurrent marginal ulcers. She presented in cardiac tamponade with a large, loculated hydropneumopericardium and pericardial fistula identified on CT imaging

She was temporized with bedside pericardial drain placement and subsequent endoscopic stenting, followed by robotic-assisted right thorascopic pericardial window and closure of the fistula.

Her post-operative course was unremarkable and she was discharged on post-operative day seven. Follow-up within our health system is currently 2.5 years.
