A Prospective, Randomized, Single-Blinded Trial of Ketamine Versus Lidocaine Infusions for Multimodal Pain Management in Traumatic Rib Fracture Patients

Abstract

Traumatic rib fractures (TFRs) carry an increased risk for hospitalization, morbidity, and mortality, it is essential to quickly initiate multimodal analgesic therapies. Both intravenous (IV) ketamine and lidocaine infusions have shown efficacy in treating patients with TRFs, however, no study has compared the effectiveness of these two agents in achieving analgesic control. We conducted a prospective, single center, single-blinded, randomized controlled trial comparing IV ketamine infusions versus IV lidocaine infusions, along with standardized adjunctive analgesia, for up to 72 hours. Adult patients diagnosed with three or more TRFs requiring hospital admission were included. Primary outcome was total opioid consumption at 24 hours using oral morphine equivalents (OME). Secondary outcomes included numeric pain scores (NPS) and OME up to 72 hours, incidence of adverse events, and hospital length of stay.
