Wait to Operate: Factors Impacting Orthopedic Trauma Operative Turnover Time (TOT)

Abstract
Background - Effective operating room (OR) management is imperative both for achieving the highest standards of patient care and optimizing hospital expenditures. Turn over time (TOT), defined as the time between the completion of one case and the initiation of the next, has the potential to significantly impact efficiency, especially in high volume trauma centers with multiple factors likely contributing. Thus, this study sought to identify factors contributing to TOT in Orthopedic Trauma Surgery.
Methods - This retrospective study utilized electronic medical records from our institution's Level 1 Trauma Center between October 1, 2023 and September 30, 2024. Data was extracted from the 24/7 dedicated orthopedic trauma operating room via a Tableau database created by our institution to examine TOT. Summary statistics were conducted using pivot tables via Microsoft Excel.
Results - 458 cases were identified with 85 distinct procedures. Initial analysis identified the five most common procedures: intramedullary rod insertion femur with cephalomedullary nail (CMN), hip hemiarthroplasty, IM rod insertion tibia, open reduction internal fixation (ORIF) for femur fracture, ORIF hip with percutaneous pinning, and the average TOT associated with each (Table 1).
Conclusion - TOT is a potentially modifiable entity for OR efficiency and operative patient care. Initial analysis determined the procedure performed to influence TOT and additional analysis is underway. Drilling down into more specific datapoints such as surgeon and anesthesiologist responsible, OR personnel, cleaning time and day of the week, will be essential to better determine the true contributing factors.
