Impact of Cocaine Use on Outcomes Post-PCI in an Urban Tertiary Care Center

Abstract

Background: Cocaine use has been identified as an important risk for adverse cardiovascular outcomes. This study aims to determine the association between cocaine use and short as well as long term outcomes post percutaneous coronary intervention (PCI). Methods: The study included a total of 2218 patients with 68 patients (3.06%) who were noted to be using cocaine while 2154 patients (96.94%) were not. Baseline characteristics included demographic data, comorbidities including prior history of coronary artery disease, cardiogenic shock amongst others were obtained. Outcomes were evaluated at 6 months and 1 year and included all cause mortality, recurrent acute coronary syndrome (ACS), need for dialysis, and need for repeat PCI. Results: The baseline characteristics of our study cohort revealed no major sex differences in cocaine use (female 2.52% vs male 3.39%, p value = 0.25). Cocaine use was most prevalent in patients with African American ethnicity (5.51%) followed by Caucasian (1.23%), the prevalence of diabetes mellitus also was significantly different (2.09% vs 3.83%, p value=0.02). Other baseline characteristics were similar across both groups. Analysis showed a higher need for dialysis at 6 months in the cocaine group (aOR 76.98, 95% CI 1.94-3043.75, p value 0.02), There was no statistically significant difference with regards to the other major outcomes studied. Conclusions: Use of cocaine was shown to increase the need for dialysis initiation at 6 months in our group. Larger studies are needed to obtain a better understanding of this association.
