Evaluation of Pharmacy Dosing Services: Vancomycin Therapeutic Drug Target Achievements

Abstract
Vancomycin is an antibiotic that is used for serious gram-positive infections. Significant adverse drug reactions include nephrotoxicity, ototoxicity, and vancomycin infusion reactions. Vancomycin requires therapeutic drug monitoring (TDM) to achieve efficacy endpoints without causing harm. At Corewell Health West Hospitals, vancomycin and its TDM follow pharmacy dosing service guidelines. On August 23, 2023, a dosing guideline update occurred in conjunction with a new operational workflow for dosing vancomycin through an electronic medical record navigator tool that standardized pharmacist data abstraction and documentation. The goal of this study was to evaluate pharmacist compliance with the vancomycin pharmacy dosing service guideline updates and the impact on safety and clinical endpoints.
